Thank you for calling our office for your chiropractic care!  Please bring your ID and insurance card, if applicable, with you.  The attached intake form can be printed and filled out before you arrive.  If you do not have access to a printer, please arrive 10 minutes prior to your appointment to allow time to fill out the intake forms.

Free parking is available behind the building in the ground level stalls.  If lot is full, go up the ramp to the third level for Stall #98.  This stall is reserved with a 20-minute parking limit. Parking directions: https://integritychirohawaii.com/parking-directions

*If there is any change to your appointment time, kindly give us 24 hrs. notice. Late cancellations and missed appointments are subject to a $25 charge.
